COMPETITIVE DIFFERENTIALS - $4.25 - 2nd, $8.00 - 3rd, $4.75 - Weekend

Case Management is a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet a patient's individual health needs through communication and available resources to promote quality, cost effective outcomes.  The Case Manager partners with the physician and other members of the healthcare delivery team to minimize fragmentation of the healthcare delivery system, ensuring that patients and families are involved throughout the process.

Minimum Education 

  • Graduate of an accredited school of nursing, BSN or MSN preferred. 

Minimum Work Experience 

  • 3 to 5 years related clinical RN experience per specialty area; i.e. Med/Surg, Emergency Care, etc. 

Required Licenses/Certifications 

  • Current Vermont RN license.  
  • Fulfills annual hospital mandatory requirements for safety, MSDS, Infection Control. 
  • BLS Certification through American heart Association.
*Required within 3 months of hire

Required Skills, Knowledge, and Abilities 

  • Knowledgeable regarding InterQual Criteria for utilization review.
  • Demonstrates safe knowledge of nursing practice skills at all times based on theory and consistent with policy and procedure.
  • Demonstrates advanced knowledge and practice of nursing.
  • Committed to assessment of own learning deficits and seeks education needed to acquire missing data.
  • Positive relationship building; effective written/verbal communication; negotiation; knowledge of contractual or risk arrangements; and ability to effect change, perform ongoing evaluation and critical analysis, plan and organize effectively and promote client/family autonomy.  
  • Desired qualities and behaviors include: collaborative team player with strong communication skills, willingness to look at whole system, problem solver and pro-active decision maker, creative, innovative, autonomous, receptive to change, and committed to professional growth.  
  • It is important for the case manager to have knowledge of funding sources, healthcare services, human behavior/ dynamics, the healthcare delivery and financing systems, and clinical standards and outcomes.
  • Basic Microsoft Windows desktop application and navigation skills. 

Pay Range = $35.46 - $60.00
